1. What documents do I need to bring to a Smart Shop to purchase a new SIM card?
One of the following (must be valid):
- National ID card.
- Passport.
- Government ID card.
- Monk ID card.
2. What documents do I need to bring to a Smart Shop to purchase a new SIM card in a company’s name?
All of the following (must be valid):
- Color copy of company patent with an original company stamp.
- Color copy of company business license with an original company stamp.
- National ID card or passport of the company’s owner (PIC of number) with an original company stamp.
- Requester’s national ID card.
3. What documents do I need to bring to a Smart Shop to change a SIM card?
I. The number is under my name:
One of the following (must be valid):
- Original or copy of national ID card.
- Original or copy of passport.
- Original or copy of government ID card.
- Original or copy of driver’s license.
- Original or copy of monk ID card.
II. The number is under a company’s name:
All of the following (must be valid):
- Authorization letter or color copy of company patent with an original company stamp.
- Requester’s national ID card.
- Requester’s passport.
III. The SIM owner is no longer alive.
All of the following (must be valid):
- Original of SIM owner’s death certificate.
- Original or color copy of SIM owner’s national ID card.
- Original national ID card or passport of requester.
- Original Family Book or Residence Book (for relatives only).
IV. The SIM owner is in hospital.
All of the following (must be valid):
- Letter from hospital proving the SIM owner is currently in hospital.
- SIM owner’s national ID card, passport or government ID.
- Original or color copy of requester’s national ID card, passport, government ID card or monk ID card.
V. The SIM owner is currently residing in another country.
All of the following (must be valid):
- Letter from the SIM owner authorizing the SIM change with the SIM owner’s thumbprint and signature (can be copies).
- Original or color copy of SIM owner’s national ID card, passport, government ID card or monk ID card.
- Original or color copy of requester’s national ID card, passport, government ID card or monk ID card.
- SIM owner must send an email to [[email protected]] authorizing the change of SIM cards. They must also attach a color copy of their Green Card or visa and passport.
VI. The SIM owner is in prison.
All of the following (must be valid):
- Letter from the authorities confirming the SIM owner is in prison.
- SIM owner’s national ID card, passport, government ID card or monk ID card.
- Requester’s national ID card, passport, government ID card or monk ID card.
4. What documents do I need to bring to a Smart Shop to transfer ownership of a SIM card to another person?
You and the intended recipient of the SIM card must go to a Smart Shop. The transfer process will cost 1 USD. You must both bring a valid national ID card, government ID card, passport or monk ID card.
